One thing to remember while applying for the card is that only primary cardholders can send an application on behalf of the applicants. If you have net banking access to your credit card, you can apply for an add-on card by logging in to your account.
You might have to fill the required details such as the applicant's name, contact details, address and identity proof, etc., and submit it.
