  2. The constituency in which the person is enrolled in the electoral roll to vote.
  3. The name of the applicant.
  4. The surname of the applicant.
  5. The part number of the voter ID of the applicant.
  6. The serial number of the voter ID of the applicant.
  7. The elector's photo identity card number.
  8. The mobile number of the applicant.
  9. The email ID of the applicant.
  10. The reason for objection or deletion of a particular detail in the electoral roll.
  11. The declaration to be signed which mentions that the applicant is in full knowledge of the process.
