Form 1A Medical Certificate for Driving Licence

In India, applying for a Driving Licence (DL) involves several steps and certain documents must be submitted. Several forms must be submitted to the Regional Transport Office (RTO) as well. In case you are 40 years and above, you must submit a medical certificate or Form 1A.

The Form 1A indicates that you are medically fit to ride or drive a vehicle. In case you are below the age of 40 years, Form 1 must be submitted. It is mandatory to submit these forms to get a DL in the country.

How to Get Form 1A Medical Fitness Certificate?

Form 1A can be downloaded from the official website of the Sarathi Parivahan (https://parivahan.gov.in/parivahan/sites/default/files/DownloadForm/Form-1AN.pdf) or by visiting the RTO. The form must be taken to a medical practitioner so that the medical examination can be conducted. Once the medical practitioner signs the form, you will need to submit it at the RTO to apply for a DL.
