Online Form Submission 

The step-by-step procedure that must be followed for Online Form Submission is mentioned below: 

Step 1: Visit the Passport Seva Online Portal

Step 2: Click on 'New User Registration'. 

Step 3: Enter the relevant details on the next page and complete the registration procedure. 

Step 4: Next, use your registered Login ID to login to the Passport Seva Online Portal. 

Step 5: Click on 'Apply for Fresh Passport/Re-issue of Passport'. 

Step 6: Fill in the relevant details and submit the application form. Please ensure that you select ‘Re-Issue of Passport’ and choose the reason as ‘Change in Existing Personal Particulars’ followed by “spouse name”.

Step 7: Next, click on 'Pay and Schedule Appointment', which can be found on the 'View Saved/Submitted Applications' screen. 

Step 8: You will have to schedule an appointment. 

Step 9: Next, the payment procedure must be completed. 

Step 10: In case of appointments at PSK/POPSK/PO, online payment has been made mandatory. Given below are the two modes by which online payments can be made: 

  1. Via Debit card /Credit Card (Visa and MasterCard) 
  1. State Bank of India (SBI) Bank Challan 
  1. Internet Banking (SBI, Associate Banks, and Other Banks) 
  1. UPI (Unified Payments Interface) 

Step 11: Once the payment is successful, you will receive confirmation of your appointment. 

Step 12: Click 'Print Application Receipt' after that. The Appointment Number/Application Reference Number (ARN) will be on the receipt. The requirement to carry a copy of the receipt has been lifted. When you visit the passport office, an SMS containing the appointment details will also be accepted. 

Step 13: Subsequently, you need to present the original documentation at the Passport Seva Kendra (PSK) or Regional Passport Office (RPO). 

Note: As per current government guidelines, a marriage certificate is generally no longer mandatory for adding a spouse name, but you should carry a self-attested photocopy of your spouse's passport if they have one. 

e-Form Submission

The step-by-step procedure that must be followed for e-Form Submission is mentioned below: 

Step 1: Download the e-Form from the official Passport Seva Kendra website.

Step 2: Click on 'Fresh or Reissue of Passport' and fill the e-Form that has been downloaded. While filling, select ‘Re-issue of passport’ under ‘Reason for re-issue', and choose ‘Change in Existing Personal Particulars’ followed by ‘Spouse Name’.

Step 3: Next, click on 'Validate and Save'. An XML file will be generated, which needs to be uploaded later. 

Step 4: Visit the official website of Passport Seva Kendra. 

Step 5: Click on 'New User Registration'. 

Step 6: On the next page, enter the relevant details and complete the registration process. 

Step 7: Next, you must login to your Passport Seva Online Portal. 

Step 8: To upload the XML file, click 'Upload e-Form' after completing the login process. It's imperative that you upload just the XML file—not the PDF. 

Step 9: Next, click on 'Payment and Schedule Appointment', which can be found in the 'View Saved/Submitted Applications' screen. 

Step 10: Next, you must schedule an appointment. 

Step 11: In case any appointments are booked at PO/POPSK/PSK, online payment is mandatory. Online payment can be completed by any of the below-mentioned methods: 

  1. SBI Bank Challan 
  1. Internet Banking 
  1. Debit/Credit Card (Visa and MasterCard) 
  2. UPI (BHIM, Google Play, PhonePe, etc.) 

Step 12: Click 'Print Application Receipt' after that. The Appointment Number/ARN will appear on the receipt. Carrying a copy of the receipt is no longer required. When you visit the passport office, you can show the SMS you received that contains the specifics of your appointment. 

Step 13: Next, visit the RPO/PSK along with the original documents. As per the latest Ministry of External Affairs (MEA) circular, a formal marriage certificate is no longer a mandatory requirement for adding a spouse’s name; a “Joint Photo Declaration” (Annexure J) is now accepted as valid proof.

Offline Form Submission 

The following are the steps to submit the form through offline mode:  

Step 1: Visit the Passport Seva Kendra official website

Step 2: Download the e-form from the website in the XML format. While the application is filled offline, the system requires the data to be uploaded in XML format.

Step 3: Fill in the form with the required details as mentioned. For adding spouse name, you will need to select “Re-issue of passport” and then under the “Change in Existing Personal Particulars”, check the spouse name box.

Step 4: Upload the XML file by logging into the website  

Step 5: Make the payment just the way done for online form submissions. Payment must made online via SBI Multi Option Payment (MOPS) including UPI, Net Banking, or cards.

Step 6: Download the payment receipt for future reference  

Following form submission, the applicant's registered mobile number will receive appointment details. To finish the procedure of adding the spouse's name to the passport, visit the passport office on the scheduled appointment day with the necessary paperwork.

Documents Required to Add Spouse Name in Passport

It is possible to add a spouse's name to an Indian passport through the Tatkal service. There will be extra fees for Tatkal services. Each applicant must pay a separate charge. MEA Allows Adding Spouse's Name to Passport without a Marriage Certificate

The Ministry of External Affairs (MEA) has announced that applicants are not required to provide a marriage certificate in order to add their spouse’s name to their passport. The main aim behind the change is to simplify the process for applicants who face difficulties in obtaining formal marriage certificates.  

In place of the marriage certificate, a self-declaration form, Annexure J, will be used. The declaration form must be signed by both parties, including a self-attested joint photograph. Moreover, the form requires the couple to declare their names, address, and marital status to confirm that they have been living together as a married couple with the place and date clearly mentioned. Aadhaar numbers, voter IDs, and passport numbers (if available), of both parties may also be needed. 

The MEA has also clarified that to remove a spouse's name and to add a new name in the passport, applicants need to provide a divorce decree, death certificate, re-marriage certificate, or a Joint Photo Declaration (Annexure J) signed by both parties, along with updated identity documents that include a QR code-enabled Aadhaar verification.
